Directions:

The HealthLink office is located on the ground floor of the Church of the Holy Name, 60 Monument Ave, Swampscott. Facing the church, HealthLink entrance is to the right, down the stairwell next to the fenced play area.

From the South - 129A North. Upon entering Swampscott take a left at the Town Monument. 2nd stop sign, take left. Church is the historic building located on the left.

From the North - 1A South. Take second left after Whole Foods Market, Walker Road. At the end of Walker Road take a right. Church the historic building located on the left.
